Social Worker - Shift Varies, Resource

This PRN position reports to the Director ofPatient Case Management. This position will fill in for staff vacations and days off. The applicant will be cross trained to areas of the facility.

Qualifications:

• Currently licensed to practice as a Social Worker within the State of Ohio.
• Bachelor’s Degree required Masters preferred. LSW required, LISW preferred
• Social Services & Discharge Planning experience preferred.

Skill and Ability:

• Ability to make psychosocial assessments and develop and implement viable care plans
• Provide direct social work intervention, transition planning and counseling to patients and their families.
• Assist patients and families in making appropriate linkages to community resources and provide consultation to those seeking input and guidance in efforts to resolve patient concerns.
• Assess the patient’s social, emotional, financial, cultural and family situation and interprets this information to the physician, nursing staff and other care team members to facilitate appropriate transition planning
• Interpersonal and analytical skills necessary to function under the current standards of Social Services.
• Computer skills that match the needs of the position.
• Physically able to perform the functions of the position.
• Maintains all required qualifications for area assigned.
• Excellent listening and communication skills
• All other duties as assigned.
