At Wayne HealthCare, our outpatient clinic, the Lifestyle Enhancement Center, focuses on prevention and control of chronic diseases and care coordination. The Lifestyle Enhancement Center (LEC) staff will work with you to make lifestyle and sometimes lifesaving changes. These changes can help to improve your quality of life and manage your disease.

Available Programs & Services:

  • Cardiac Rehab: This individualized program assists patients with managing symptoms of heart disease while decreasing the chance of (additional) heart attacks. The goal is to help patients improve their endurance and strength and educate them on how to lead a healthier lifestyle. Benefits of the program include a personalized exercise program, patient education and support, and much more.
  • Pulmonary Rehab: Patients with pulmonary problems such as chronic obstructive pulmonary disease (COPD) may benefit from our Pulmonary Rehab program. Benefits of pulmonary rehab include an evaluation and instruction of physical activity, a monitored and supervised personalized exercise program, and a chance to meet and share stories with others.
  • Peripheral Vascular Disease Exercise Therapy: This slow progressive circulation disorder is caused by the narrowing or blocking of a blood vessel. The LEC helps patients with exercise therapy to control PVD symptoms and halt its progression. With customized treatment, PVD patients lower their risk of a heart attack, stroke, and many other complications.
  • Transitional Care / Care Coordination: Transitional Care may be utilized upon discharge if you have a medical condition that requires a stay at our hospital. If you have complex medical needs, our Transitional Care program will focus on preventing or reducing the symptoms of your condition through individualized comprehensive care. Participating in Transitional Care allows individuals to manage their own health better, prevent possible complications, and re-hospitalization.
  • Blood Pressure Clinic: The goal of our clinic is to identify and monitor blood pressure to prevent other organ damage from untreated or under-treated hypertension (high blood pressure). This free service is offered to community members with no appointments required.
  • Tobacco Cessation Programs: Quitting smoking is anything but easy, but our staff is committed to educating and helping our community take a step toward healthier living through the Freshstart Tobacco Cessation Program. Smokers who want to quit are much more successful when they take advantage of our program rather than doing it alone.
  • Diabetes Services: Diabetes is a chronic (life-long) condition that affects how the carbohydrates we eat are metabolized, resulting in blood sugar levels that are higher than normal. Chronic high blood sugar can damage vital organs without prompt treatment, which is why it’s so important to be educated and learn proper lifestyle changes to maintain good blood glucose control and prevent the effects of long-term complications. The programs we offer include diabetes education program, (pre)/diabetes group classes, individual sessions with a Certified Diabetes Educator, individual sessions with a registered dietitian, and more.
