Social Worker - Multiple Positions

Open Social Worker Positions:

(1) - Full Time - 1st Shift - working 8 hour shifts, 40 hours per week*

(1) - Part Time - 1st Shift- working 8 hour shifts, 32 hours per week*

The Social Worker provides direct social work intervention, transition planning and counseling to patients and their families. The applicant will be cross trained to other areas of the facility. This position reports to the Director of Patient Case Management.

Qualifications:

• Currently licensed to practice as a social worker within the State of Ohio.
• Bachelor’s Degree required Masters preferred. LSW required, LISW preferred
• Social Services & Discharge Planning experience preferred.

Skill and Ability:

• Interpersonal and analytical skills necessary to function under the current standards of Social Services.
• Physically able to perform the functions of the position.
• Maintains all required qualifications for area assigned.
• Ability to make psychosocial assessments and develop and implement viable care plans
• Excellent listening and communication skills
• Assess the patient’s social, emotional, financial, cultural and family situation
• Interpret information to the physician, nursing staff and other care team members to facilitate appropriate transition planning
• Assist patients and families in making appropriate linkages to community resources
• Provide consultation to those seeking guidance in efforts to resolve patient concerns
• All other duties as assigned.
