Bridging Health Care Gaps…No Matter What

At Wayne HealthCare, we offer Transitional Care to help people with complex medical needs. Our Transitional Care program helps patients focus on preventing or reducing symptoms while managing medical problems through individualized comprehensive care. Whenever medical conditions require a hospital or other facility stay, the Transitional Care program may be utilized upon discharge.

Participating in Transitional Care allows individuals to manage their own health better, prevent possible complications, and re-hospitalization. The program is well-suited to critically ill seniors with complex chronic conditions, and to help their family caregivers. The primary benefit of the transitional care program is to coordinate continuity from hospital-provided care to a different level of care at another facility or the patient’s home. The goal is to lower the odds of readmission to the hospital.

Depending on a patient’s condition, transitional care may include the following:

  • Rehabilitative care
  • Physical, occupational, or speech therapy
  • Intravenous therapy
  • Frequent lab tests
  • Wound care
  • Fall prevention
  • Nutritional counseling
  • Respiratory therapy monitoring
  • And more
