Wayne HealthCare invites you to participate in our community-wide health survey. The information collected in this survey will help us better understand your concerns, challenges, and needs related to health to improve the quality of care, increase access to care and keep all members of our community healthy.
All participants will remain anonymous and have an opportunity to enter a drawing to win one of two $100 Amazon gift cards.
The survey must be completed by May 30, 2021
